This notice describes how medical information about you may be used and disclosed by Elevate Health and how you can get access to it. Please review it carefully.

## How we use PHI

We use protected health information (PHI) to provide treatment, obtain payment for services, and run our practice (health-care operations). We may share PHI with other clinicians involved in your care.

## Permitted disclosures

-   As required by law or public-health authorities 
-   To business associates who have signed HIPAA agreements 
-   To family or friends you have authorized in writing 
-   For appointment reminders and treatment alternatives 

## Your rights

-   Inspect and receive a copy of your record 
-   Request amendments to your record 
-   Request confidential communications by alternate means 
-   Request restrictions on certain uses and disclosures 
-   Receive an accounting of certain disclosures 
-   Receive a paper copy of this notice on request 

## Our obligations

We are required by law to maintain the privacy of your PHI, provide this notice, and notify you following a breach of unsecured PHI. We must follow the terms of the notice currently in effect.

## Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
